Hospitalization can be difficult for some patients, especially the elderly or those with multiple chronic conditions. A new program, Hospital at Home, now enables eligible patients to have hospital care in the comfort of their own home.
Holy Name Medical Center is the first hospital in New Jersey to offer Hospital at Home, a program approved by the Centers for Medicare and Medicaid Services (CMS), that allows health care services to be provided outside a hospital setting. Patients who require treatment for many acute conditions, such as asthma, congestive heart failure, pneumonia and others, can receive care through proper monitoring and treatment protocols without being confined to a hospital.
Patients in the program will be evaluated throughout the day, either in person or remotely by a registered nurse, doctor and/or nurse practitioner. Providers are on call 24 hours a day, seven days a week. The program also provides treatments such as oxygen therapy, intravenous medications as well as diagnostic testing.
Hospital at Home is a way to receive hospital care while enabling patients to spend time with family and caregivers without the visitation restrictions of a hospital. At any time, patients have the option to transfer to the hospital.
