Overview

Supportive care is a team-driven, multidisciplinary model of care given to any patient with a serious illness. It focuses on patient and family outcomes to improve quality of life, symptoms and emotional and spiritual well-being. It also addresses caregiver needs.

Holy Name provides supportive care for patients with a progressive, life-limiting illness, disease or condition that significantly impacts their quality of life, though they may live for decades. The interdisciplinary team includes nursing and medical specialists, pharmacists, social workers, neuropsychologists, and bereavement and spiritual counseling professionals.

Under the supportive care umbrella, Holy Name also offers palliative and hospice care.

Palliative care is provided to patients who are hospitalized or those living at home with a serious condition. It is appropriate for patients at any age and during any stage of illness. It may accompany curative treatment and help with symptoms such as pain, nausea, vomiting, anxiety, shortness of breath, insomnia, constipation, diarrhea, fatigue and others.

Hospice care is a multidisciplinary program that provides personalized and compassionate care for patients with a life-limiting illness, which no longer responds to treatment. It is designed to enhance the quality of life for the patient and their loved ones. Holy Name offers hospice care in the patient’s home or through its hospice center, Villa Marie Claire in Saddle River, NJ.
