Holy Name, in collaboration with the Community Health Improvement Partnership of Bergen County, is conducting a survey to assess community health needs and asking residents to participate. The triennial survey asks participants to share their thoughts on how Holy Name can best meet their needs to boost their health and well-being. The Community Health Needs Assessment survey can be accessed here: www.prcsurvey.com/begin/bergen2022chna. The link will be active until August 19, 2022.
“Holy Name is there for our community, not just when you are seeking care, but as your health partner to help you lead a healthy lifestyle. This survey allows us to hear directly from our community about what they need to achieve their health goals,” said Cedar Wang, APN, Vice President of Nursing at Holy Name. “We’re proud to offer robust community outreach that ranges from support groups to educational programs to a mobile education lab and the survey will allow us to create additional programs to improve health.”
The Community Health Needs Assessment will be distributed through Professional Research Consultants on behalf of the Community Health Improvement Partnership of Bergen County, a collaboration of Bergen County hospitals and public health departments. The survey findings will allow each member to then implement solutions to monitor population health and determine which services are needed within their region. These findings help to establish health improvement priorities and inform how outreach teams can best serve local communities.
