Overview

Oncology Social Worker

The comprehensive offerings of the Patricia Lynch Cancer Center include consultation with a social worker focused on understanding the needs of oncology patients and addressing any possible barriers to care. The Cancer Center's certified oncology social worker delivers outstanding psychosocial care to patients, in addition to their families and caregivers, based on individual needs and circumstances.

The role of Holy Name's oncology social worker encompasses a broad range of services:

  • Assessing and responding to patients in crisis and/or distress.
  • Helping patients access practical assistance, when needed.
  • Coordinating patient transportation.
  • Linking the patient to any needed programs and services, such as palliative care, hospice, home care, behavioral health/addiction services, financial assistance programs and fertility preservation.
  • Processing referrals/orders for durable medical equipment, such as a wheelchair, hospital bed, cranial (hair) prosthesis, etc.
  • Addressing barriers to care, including homelessness, food insecurity, inadequate insurance coverage, etc.
  • Guiding patients through the process of applying for various aid, including social security disability and medical insurance, and general assistance through the County Board of Social Services, co-pay assistance, etc.
  • Facilitating communication between patients and their treatment team
