Improving the health & wellness of our communities
Holy Name is committed to enhancing the health of the people we serve. To improve individual and population health, we need to first measure the health status and needs of our community.
We proudly participate in the Community Health Improvement Partnership of Bergen County, a collaborative of Bergen County hospitals and the public health department to conduct a Community Health Needs Assessment. The assessment, which takes place every three years, evaluates the health needs of individuals living in our region. The purpose of this assessment is to identify major health problems, gaps in services and other factors which may contribute to less than optimal health status for our community members. Holy Name uses this information to develop strategies and implement programs that address many of the health concerns impacting residents.
