To Make a Referral to Holy Name's Palliative or Hospice Care, please email the following to: supportivecare@holyname.org

Insurance information
History and physical/progress notes/any pertinent health information
List of current medications
Reason for referral/type of visit (symptom management, GOC, MMP, psychosocial support)/hospice referral order
Clinic/telehealth
Referral contact phone/email

Palliative: the office will call the patient and schedule an appointment. The Palliative Care Clinic number is 201-379-5610.

Hospice: Hospice intake staff will contact the referral source to collaborate prior to contacting the patient or patient's family, unless otherwise instructed. Please call the main hospice number: 201-833-3188.
