Nurse Navigator

A breast cancer diagnosis can lead to a lot of questions and require many decisions. A nurse navigator, familiar with the challenges that occur during the breast cancer journey, provides valuable education and support to patients and their loved ones. She listens to each patient's concerns and offers guidance, reassurance and access to resources and referrals. She supports patients and their families through diagnosis, treatment, symptom management, recovery and beyond into survivorship.

Whether patients are newly diagnosed with breast cancer, currently in treatment, or have had a recurrence, Holy Name's breast cancer nurse navigator remains a constant presence and assists with the patient's social, emotional, and educational needs. She will:

  • Provide emotional and psychological support for patients with breast cancer and their families, to help them understand and cope with complex feelings and fears that may accompany their diagnosis.
  • Advocate for the patients, families and caregivers to overcome healthcare barriers, including, but not limited to, social, financial, and/or physical obstacles.
  • Facilitate the medical team's recommendations regarding the patient's care and treatment plan, explaining options and outcomes, and answering any questions.
  • Assist with scheduling appointments, coordinating follow-up care, and guiding the patient through the complete treatment plan.
